FAQs
Q: How is wellness case management different from regular health coaching?
A: Health coaching focuses primarily on personal wellness goals, behavior change, and lifestyle habits. Wellness case management takes a broader approach by also addressing healthcare navigation, community resources, social determinants of health, and barriers that may prevent you from achieving your goals.
Q: What types of resources can you help me find?
A: Depending on your needs and location, we can explore healthcare services, community wellness programs, food assistance, transportation, housing support, benefits information, and other resources that may improve your well-being.
Q: Can you help if I have multiple chronic health conditions?
A: Yes. I can help with general self-management education, organizing wellness priorities, identifying practical support needs, and implementing lifestyle recommendations from your healthcare team.
Q: Can you coordinate with my doctors or other service providers?
A: When appropriate and separately agreed upon, I may help facilitate nonclinical communication and resource coordination with your permission. This requires appropriate authorizations and does not replace clinical care coordination performed by licensed providers.
Q: Can you help with Medicaid, Medicare, or other insurance resources?
A: I can help you understand general program information, identify appropriate contacts, and organize questions regarding eligibility or available services. Coverage and eligibility decisions are made by the relevant agencies or insurers.
Q: Will I receive a personalized plan?
A: Yes. The consultation includes a brief action summary outlining your priorities, initial resource options, wellness goals, and practical next steps.
Q: Can I receive continuing case management services?
A: Yes. Additional appointments or separately agreed ongoing support arrangements may be available depending on the level of assistance needed.
Q: Does this include completing applications and contacting organizations on my behalf?
A: The initial session primarily focuses on assessing needs, identifying resources, and planning next steps. More extensive coordination, application assistance, and agency follow-up require a separate scope of service.
Q: Is this covered by health insurance?
A: This service is listed at a self-pay rate. Insurance coverage or direct billing is available only if an applicable payer contract and service eligibility have been established. (Coming soon for some states)
