This visit is intended to review results or treatment recommendations from a recent primary-care appointment, assess your response to a care plan and discuss next steps. For ongoing management of chronic conditions, select our Chronic Disease & Medication Management service.

This is a follow-up appointment, not a new-patient evaluation. Patients must be physically located in New York during the visit. Additional testing or an in-person evaluation may be recommended when appropriate.

Self-pay follow-up: $100. Medication, laboratory testing and other outside services are separate.
