Completed intake form and consent required. This service provides wellness coaching, education, and resource navigation-not medical diagnosis or treatment. Clients are encouraged to maintain communication with their healthcare providers.
Navigation may include helping clients prepare questions, locate resources, understand healthcare-system process, and organize survivorship goals.
