DOT-SAP Evaluation

Service Description

The DOT SAP process is designed to help employees who have received a DOT drug or alcohol violation understand what is required before they can return to safety-sensitive duties. You are billed a flat rate of $300 for the entire process.

1. Initial SAP Evaluation
The process begins with a confidential evaluation with a qualified Substance Abuse Professional (SAP). During this appointment, we review the circumstances surrounding the violation, relevant history, and any factors that may affect safe performance of safety-sensitive duties.

2. Personalized Recommendations
Based on the evaluation, the SAP determines whether education, treatment, or other services are appropriate. Recommendations are individualized to the employee’s circumstances and are provided in writing. This may include educational classes, treatment, or 12 step meetings or a combination.

3. Completing the Recommendations
The employee completes the education, treatment, or other services identified by the SAP. The SAP does not provide treatment simply as a formality. The recommendations are based on the evaluation and the requirements of the DOT return-to-duty process. There is no time limit for completion but highly encouraged to in order to return to work.

4. Follow-Up SAP Evaluation
After the recommended services have been completed, the employee returns for a follow-up evaluation. The SAP reviews documentation showing completion of the recommendations and determines whether the employee has demonstrated successful compliance with the SAP process.

5. Return-to-Duty Process
When the SAP determines that the requirements have been satisfied, the required SAP information is entered into the appropriate DOT process so that the employer and/or other authorized parties can proceed with the return-to-duty requirements.

6. Follow-Up Testing
After a successful return-to-duty process, the employee may be subject to a DOT follow-up testing plan. The SAP establishes the follow-up testing schedule when required, while the employer is responsible for implementing the testing program.

The SAP process can feel complicated. My role is to make each step understandable, professional, and respectful while maintaining the standards required by the Department of Transportation.
Accountability and support can exist together. The goal is a clear path forward.

Service Requirements

Please provide the following information and documentation before your scheduled DOT SAP evaluation:
Required Information

* Full legal name and current contact information
* Date of birth
* Employer or prospective employer information
* Job title or type of safety-sensitive position
* Description of the DOT drug or alcohol violation
* Date of the violation
* Type of test involved, if known
* Substance identified, if applicable
* Whether you have received a DOT Clearinghouse notification regarding the violation

Documents to Provide, If Available

* Copy of the employer’s violation or removal-from-duty notice
* Drug/alcohol test result or other documentation related to the violation
* Any SAP or treatment documentation from a previous provider
* Documentation of education or treatment already completed
* Employer or Medical Review Officer (MRO) documentation, if provided

Before Your Appointment
Please complete all requested forms and provide accurate information. If you do not have a particular document, simply indicate that it is unavailable. Do not delay scheduling your evaluation because you are missing paperwork.

You will also need to be prepared to participate in a confidential, professional interview regarding your substance use history and the circumstances surrounding the DOT violation.

Important: The SAP evaluation is not a treatment session. Its purpose is to conduct the required professional evaluation and determine appropriate recommendations under the DOT return-to-duty process.
