ADHD Medication Management - Includes initial evaluation, assessment and treatment - New York State only

You are welcome to book this consultation if you are seeking evaluation for possible ADHD, or if you have ADHD and are seeking treatment.
Please note that you MUST confirm you are physically located in New York State at the time of the consultation.
This appointment covers our initial evaluation, assessment and a follow up appointment all within the first month. Follow up appointments are $125 and are needed monthly if controlled substances are prescribed.

Please gather any records you may have regarding your ADHD diagnosis, symptoms, and previous or current treatment. If you have not been formally diagnosed and/or treated for ADHD please note that.

Services are available to eligible adults and vary by state. Booking does not guarantee treatment or prescription medication. Clinical recommendations are based on an individual medical evaluation.

Please do not submit medical history, medical records, laboratory results, medication information or other sensitive health information through Sofia Health messaging. After booking, you will receive instructions for completing your secure clinical intake.

Q: Do you prescribe medication for ADHD?
A: Yes, if appropriate and supported by your documentation and evaluation.

Q: Am I required to see you for follow up appointments?
A: We will have a follow up appointment every month or so. Controlled substances are not prescribed for more than a month at a time.

Q: Do you prescribe controlled substances for ADHD?
A: Yes, if clinically indicated. We will meet at least twice via video appointment, and I will also follow up with you in between your initial visits with a series of questionnaires designed to assess your concerns.
