During this initial consultation, we will thoroughly analyze your complete medical history, lifestyle factors, environmental exposures, and advanced functional lab results (including gut microbiome, advanced thyroid panels, cellular nutrient levels, and systemic inflammation markers). Together, we will co-create a highly personalized, systems-based therapeutic roadmap designed to restore your body's natural biochemistry and optimize your long-term vitality.

Please upload any recent blood work or laboratory reports completed within the last 6 months.
Please list all current prescription medications, over-the-counter drugs, and dietary supplements you are taking (including dosages).
Please complete and submit our comprehensive Functional Medicine Intake Questionnaire at least 48 hours prior to your scheduled session.

Q: Are laboratory testing fees included in the consultation price?
A: No. The consultation fee covers Dr. Goel’s comprehensive clinical review and personalized plan development. Advanced functional lab testing is billed separately, though we work to secure direct physician-pricing discounts for our patients.

Q: Do you accept health insurance?
A: Prime Vitality Care operates as a direct-pay specialty clinic to ensure we can provide unrestricted, high-quality care. We do not accept insurance directly, but we can provide you with a detailed superbill (coded invoice) that you may submit to your insurance provider for potential out-of-network reimbursement. HSA/FSA cards are gladly accepted
