We’ll also review any recent lab work (or recommend specialized testing if needed) to get a complete picture of your hormonal health. From there, you’ll receive a personalized plan that may include lifestyle strategies, nutrition guidance, targeted supplementation, or treatment options designed to restore balance and help you feel like yourself again

Service Requirements

To ensure your consultation is as thorough and personalized as possible, all new patients must complete intake forms through our secure electronic health record (EHR) at least 24 hours prior to your appointment.

You’ll be asked to provide:

Current symptoms and health concerns (fatigue, weight changes, sleep issues, mood, libido, etc.)

Complete medical history, including surgeries, chronic conditions, and past treatments

Current medications, supplements, or hormone therapy (if applicable)

Recent or past lab results (bloodwork, hormone panels, thyroid labs, etc.)

Family history of hormone-related conditions (thyroid disease, diabetes, PCOS, menopause/andropause, etc.)

Lifestyle details such as nutrition, exercise, sleep, and stress management

Primary care provider or specialist information (if applicable)
