Key Components:

Initial Assessment (2 Hours): A deep dive into your health history, lifestyle habits, and personal goals. Together, we’ll create a customized wellness plan based on your unique needs and aspirations, establishing the foundation for your personalized wellness plan.

Specialized Lab Testing: Access an advanced panel of biomarker tests to establish a baseline and track progress at regular intervals throughout the program. Lab tests are conducted through a partnered provider.

Service Requirements:

Consent Form and Acknowledgment: All clients must review and sign the consent form before the program begins. This document outlines the scope of services, confirms that no medical diagnosis or treatment will be provided, and acknowledges understanding of program policies.

Client Intake Form: Clients are required to complete a detailed intake form covering their current health, relevant medical history, and personal wellness goals. This form ensures that the initial assessment is productive and tailored to the client’s specific needs.

Lab Testing Registration: Clients must register for the partnered lab testing program within 30 days of the initial assessment to ensure timely progress within the program.
